How long is it after you eat until you go to bed?

I am wondering about the correct way to sign a doctor's question: Have you had a tetanus shot within the past five years?
I think that a Topic Sentence would be best. However, when I sign the sentence, it does not look right to me:
SHOT T-E-T-A-N-U-S. FIVE YEAR PAST. UP-TILL-NOW. YOU GET?
In addition, I am wondering: Do you GET a shot? Or do I use a different sign?
The "within" aspect of the question has me wondering: The concept "within" is often used (within certain parameters, within jurisdictions, within time frames, etc.). Is there a sign for the "within" concept?
Any advice that you can give me will be greatly appreciated.

How about this:
UP-TO-NOW 5-YEAR SHOT PREVENT T-E-T-A-N-U-S YOU?

I wouldn't use "GET" since that sign has somewhat of a connotation of "receiving something tangible."
